The Oklahoma Advance Directive decrees a formal arrangement for individuals to map out a predetermined course of action regarding health care decision-making for themselves. The document is broken down into four sections containing a framework for the principal’s selections of how medical efforts will be approached if situations arise where they can no longer communicate their desires. Part one is designed to record one’s wishes regarding terminal illness and life-sustaining treatments, allowing physicians to follow the direction of the patient’s needs. The secondary portion of the paperwork serves to assign an attorney-in-fact to make selections on behalf of the declarant in the event that they are deemed unconscious for an extended period. The final parts of the contract address organ donation and confirm legal obligations to instate the contract. Before signing, seeking legal advice is common practice to ensure all aspects of the transaction are considered before the commitment is established.

Definition – “Advance directive for health care” means any writing executed in accordance with the requirements of Section 3101.4 of this title and may include a living will, the appointment of a health care proxy, or both such living will and appointment of a proxy (§ 63-3101.3(1)).
Signing Requirements – There must be two (2) qualified witnesses made available to view and attest to the signing of the document to render it legally enforceable (§ 63-3101.4(A))
