The Connecticut Living Will is the predetermination of what is to occur if the declarant ever winds up in a position where they are incapacitated and facing serious medical problems. This document allows users to list their wants concerning resuscitation measures (CPR), life-sustaining treatments (artificial respiration, nutrition, & hydration), and pain medication. Individuals interested in fulfilling this written instrument must provide their name as the author of the document, make their selections, and sign the form according to state law.
Laws
Definitions – “Living will form” means a written statement in compliance with section § 19a-575a, containing a declarant’s wishes concerning any aspect of his or her health care, including the withholding or withdrawal of life support systems (§ 19a-570(9)).
Signing Requirements – Acknowledgment of two (2) witnesses is needed for proper enforceability(§ 19a-575a). The form is advised to be corroborated by a licensed notary (§ 19a-578).
